Eligibility: Older adults 55+ living with mental health and/or substance use issues, cognitive issues (i.e. dementia), frailty, and other psychogeriatric conditions related to the aging process. Individual resides in East Toronto (Yonge Street to the west, Eglinton Avenue to the north, Lake Ontario to the east and south).
Delivery: A psychogeriatric specialist (i.e. a registered social worker/psychotherapist) provides goal-oriented, time-limited supports to individuals in person in East Toronto and/or over the phone/virtually. Service delivery is approximately once a week for up to 9 months with the possibility of extension to 18 months.

Program description

The Social Work and Full Circle programs offer individual, clinical supports to people aged 55 and older living with mental health and/or substance use issues, cognitive issues (i.e. dementia), frailty, and other psychogeriatric conditions related to the aging process. Psychogeriatric specialists help clients to problem solve and learn new strategies to take care of one’s health and improve well-being.

 

The Social Work team offers one-on-one support including case management, therapeutic counselling, and information and referrals for services and supports. Services can be provided for up to 6 to 9 months. Clients who require longer support may be referred to the Full Circle program, run by a team of psychogeriatric specialists who deliver services for up to 12 to 18 months.
