1117 Gerrard St. E. offers mixed Rent-Geared-to-Income and Market Rent units for seniors 59+. This site has a total of 36 one-bedroom independent living units with open-conceptlayouts. Residents have access to a part-time housing worker for tenancy supports and community programs.
Application information
Applications must be accompanied by proof of Status in Canada i.e., birth certificate, Canadian passport, Permanent Resident card, Refugee Claimant form) and most recent Notice of Assessment (NOA).
All Rent-Geared-to-Income unit applications must be completed through the Coordinates Access, PATHS program, with the City of Toronto.
All market rent applications can be submitted to our Housing Access Centre, with the appropriate supporting documents.

A transitional housing program for men 55+ with a history of homelessness and mental health or addictions issues. We provide safe housing and wrap-around supports to help clients build new skills and recreate a sense of community.

2 Laxton Ave. is a mid-rise apartment building with 52 units. There are 22 studio and 30 one-bedroom units. New units will be administered through the COHB Program and community referrals.

Operated in partnership with Fife House, this site has 112 mixed units and offers supportive housing for Fife House clients living with HIV/AIDS and frail seniors receiving WoodGreen assisted living services. It also hosts an 11-unit transitional housing program for individuals living with HIV/AIDS and working toward permanent housing.
These programs offer individual, clinical supports to people aged 55 and older living with mental health and/or substance use issues, cognitive issues (i.e. dementia), frailty, and other psychogeriatric conditions related to the aging process. 
Our team delivers hot and frozen meals to seniors and adults living with disabilities. We also provide at-home security and wellness checks and help with accessing local services.

On-call crisis intervention and outreach for older adults (65+) with dementia, addictions or mental illness. Our response team is available daily from 9 a.m. – 5 p.m.

We offer supervised group activities for dependent adults living with Alzheimer’s, disabilities or frailty. On-site support services include leisure activities, meals and personal care.
Offered in partnership with Neighbourhood Link, this program helps people obtain basic identification to access health care, sources of income such as pensions, and other services.
